GUIDELINES FOR AGENCIES PROVING MENTAL HEALTH SERVICES/CASE MANAGEMENT IN THE IBERVILLE PARISH SCHOOLS
Mental Health services are available to students and their families within the community and the surrounding area. These agencies provide counseling, therapy and other services to address problems being experienced by students in our schools. These services are available in addition to specific services the school district must provide under the Individuals with Disabilities Act (IDEA) and Section 504 of the Rehabilitation Act of 1973.
The Iberville Parish School System has developed the following guidelines to help establish a collaborative relationship with community based mental health agencies.
An agency must send a written request to the Supervisor of Child Welfare and Attendance to establish a cooperative agreement with the Iberville Parish School System.
An agency or individual must submit the following information. (Documentation must be submitted no less than thirty (30) days prior to services being rendered. This documentation shall be submitted annually.)

Mental Health Clinicians must be a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Psychologist, Licensed Psychiatrist, or a master’s level professional working towards licensure under a licensed clinician.
Certification of insurance (i.e. professional liability, worker’s compensation)
Documentation of fingerprinting and background checks (expense incurred by agency)
Copy of state license to operate as a non-profit organization.
List of employees who are also employed by the Iberville Parish School System shall be provided so that the school system can monitor violations of ethics code.
A program design of services to be rendered/offered.
Documentation must be submitted no less than thirty (30) days prior to services being rendered. This documentation shall be submitted annually.
No agency or individual may provide mental health care services to the public schools of Iberville Parish until the above requested information has been verified and written permission to administer services is given by the Supervisor of Child Welfare and Attendance and the Superintendent.
All agency personnel must submit documentation in advance of scheduling services. Agencies must provide the school with written parental consent for services and for review/copy of records prior to rendering services and /or receiving records. Agency personnel must sign in at the school office and wear an identification badge (provided by agency).

Only one agency may observe a class at any given time. Agencies must provide the school with written parent consent for services and for review/copy of records prior to rendering services and/or receiving records.
Principals shall have the discretion as to the number of service providers on their campuses at any given time.
All agencies/providers must specify services to be provided and agree to indemnify, defend and hold harmless the Iberville Parish School System and its agents and employees from any and all losses, damages, expenses or other liabilities, including but not limited to attorney fees, court costs and other costs of defense, arising from or in any way connected with any claim of personal injury, malpractice, death, property damage or any other liability that may be asserted against the Iberville Parish School System or its agency/private provider, his/its employees, contractors, or agents in performing its obligations.
Oversight of all agency/provider activities shall be the responsibility of the principal.
